Here are some general guidelines for submitting insurance requests for locations or equipment:

Insurance forms can be obtained in the Arri Resource Center or online from the CTVA website.

Insurance requests must be submitted at least two (2) weeks in advance. They must be submitted in person to Michael Bryant in MZ 191 and cannot be dropped off with any of the Resource Center personnel or left with any of the office staff in MZ 195.

Once insurance is processed, coverage is valid for the listed equipment and/or location, and will be valid only for the dates indicated on the insurance forms.

Insurance is available for CTVA 452/442/441/355/341

Any changes in equipment listed or production dates must be resubmitted in order for insurance certificates to be valid.

In the event of loss or damage and an insured individual needs to process a claim, there is a standard $1000.00 deductible. In addition, the individual needs to provide a Police Report that documents the events that occurred leading to the loss or damage of the property.

The university and the CTVA department do not provide Workers’ Compensation or Vehicle Insurance. These must be obtained from outside vendors.
